For those individuals or groups wishing to open a Family Day-Care business in their homes or a Day Care Center, all applications must be submitted to the Connecticut Department of Public Health Day Care Licensing Program for approval. The link below will direct you to the required forms and application of the DPH Day Care Licensing Program. Application should also be made to the Brookfield Health Deparment. Environmental Inspection by the Brookfield Health Department are required to be performed every two years at Day Care Centers only.
*Please note: Approval of your business may also be required by other departments in the Brookfield Town Hall such as Zoning and/or Building, please contact them as well.
