Aging in Place and Other Housing Options Panel Discussion Wednesday, May 1 at 1:30 p.m.

Join us in this informative community panel discussion with industry experts! The panel will include: • Peter Crossett from SYNERGY HomeCare - Home Care Agency • Danielle Ramos from Bethel Health Care – Assisted Living and Rehab • Marty Schwartz from Oasis Senior Advisors – Senior Home Care Solutions • Jennifer Zinzi from Keystone Place at Wooster Heights – Continuous Care Community. Alzheimer’s and Dementia Caregiver Support Group Monday, May 20 at 11:00 a.m.

This confidential group will be meeting the last Monday of each month at 11:00 a.m. at the Brookfield Senior Center. These groups are a safe place for caregivers and loved ones of people with dementia to exchange information, share feelings and to develop a support system. Please register so we know how many to expect.

Join Danielle Ramos, Bethel Health Care, for a fun hour of memory joggers and trivia. Preventing forms of dementia is largely driven by lifestyle and the choices we make daily. Make the right choice - your brain will love you for it! If there are enough people, we may place you on teams. As always, Danielle will bring along a healthy mid-morning snack for you to enjoy. CONNECTICUT HOMECARE PROGRAM FOR ELDERS (CHCPE)

To be eligible, applicants must be 65 years of age or older, be a Connecticut resident, be at risk of nursing home placement and meet the program’s financial eligibility criteria. To be at risk of nursing home placement means that the applicant needs assistance with critical needs such as bathing, dressing, eating, taking medications, toileting. The CHCPE helps eligible clients continue living at home instead of going to a nursing home. Each applicant’s needs are reviewed to determine if the applicant may remain at home with the help of home care services.

CONNECTICUT STATEWIDE RESPITE CARE PROGRAM

This program provides relief to stressed caregivers by offering information, support, care planning, and services for individuals with Alzheimer's Disease or related dementias. Clients may receive care from agencies (traditional care option) or hire someone personally (self-directed care option). The program can subsidize service costs up to $7,500 annually, contingent on available funds and the applicant's need level as assessed by the Care Manager, with a limit of 30 days of out-of-home care. Respite care services (excluding Adult Day Care) available per year to each applicant. This program is a joint partnership between the Alzheimer’s Association Connecticut Chapter, the Area Agencies on Aging, and the Connecticut Department of Aging and Disability Services - State Unit on Aging.   

The eligibility criteria are as follows: 

Effective from July 1st, 2023, applicants with Alzheimer’s or related dementia must have an annual income of $55,561 or less and liquid assets not exceeding $147,715, and they must not be participants in the Connecticut Homecare Program for Elders. Income includes Social Security (excluding Medicare Part B premiums), Supplemental Security Income, Railroad Retirement Income, veterans' benefits, and any other one-time or recurring payments. Liquid assets encompass checking and savings accounts, stocks, bonds, IRAs, certificates of deposit, and any other assets readily convertible to cash.
A Co-payment of 20% of the cost of services is required unless waived by the Agency on Aging Care Manager due to financial hardship.  

An application must be completed which includes a Physician’s statement certifying the condition of the individual with Alzheimer’s disease or a related dementia.
