Submit All Required Information Listed Below:
1. Completed Food Establishment Application (Fill in ALL blanks)
2. License fee payment for your classification (See chart below)
3. Copy of current Certified Food Protection Manager Certificate(s) and/or training documents for ALL food preps (Training forms available in our office and online)
4. Private well water report (If applicable)
5. New menu
6. Completed/Updated Emergency Contact form
NOTE: Any incomplete or missing information will delay licensing process.
LICENSE FEE SCHEDULE
Class 1: $150, Class 2: $250, Class 3: $300, Class 4: $350
LATE FEE (Applied 10 days after expiration): Standard Fee + 50%

BODY CARE LICENSE FEE SCHEDULE
First 3 stations, bay, chairs or individual service rooms: $70
Each additional beyond 3: $20 (Max is $250)
*PLEASE SUBMIT PROOF THAT REAL ESTATE AND PROPERTY TAXES ARE PAID IN FULL. RECEIPT ATTACHMENTS ARE ACCEPTED.
*APPLICATIONS WITHOUT THE ABOVE LISTED REQUIREMENTS WILL DELAY LICENSING.
